Council invites you to complete this bicycle crash survey form (below on this page) if you:
witnessed a bicycle crash or near miss involving another vehicle such as a car, truck or motorcyclist;
were involved in a bicycle crash as a rider or as the driver of another vehicle; or
wish to complete the form on behalf of the cyclist involved.
We will collate the information provided by the public to:
monitor the number of bicycle crashes in the Coffs Harbour Local Government Area;
inform relevant authorities;
assist with funding applications; and/or
highlight 'blackspots' for cyclists in the area.
