These application forms can be used to request access to Council documents.

Access Information Form (Informal) (112KB)
You can ask us for specific information. Council aims to release information without the need for a formal application, unless there are good reasons to require one. Use this form to make it easier for Council to find the information you are seeking.

Access Information Form (Formal Consent) (127KB)
If you haven't been able to get the information you need in any other way. You can formally request specific information. In limited circumstances, Council will require a formal application for access to certain information.

Right of Internal Review Form (97KB)
Applicants have the right to have decisions made by the RTI Officer reviewed.  Applications need to be lodged within 20 working days after the notice of the decision is given or (in the case of the deemed refusal by Council to deal with an application) more than 20 working days after the deemed refusal.
